LICENSED PRACTICAL NURSE

Employee Type PRN Description CRHC is looking for a PRN (as needed) Licensed Practical Nurse to join our team in the Clinics. This position is needed to help out between the Specialty, Mental Health, and Physicians Clinic. Hours and days will vary depending on department needs.
PRIMARY FUNCTION
Responsible for Advancing Exceptional Care by prioritizing Safety and Quality in every aspect of patient care. Education and/or Experience Graduate of accredited practical nursing program Certificates, Licensure, Registrations Currently licensed as a Licensed Practical Nurse in the State of Iowa or compact state The facility operates 24/7; however, work schedules may vary based on departmental needs to ensure the highest standard of patient care and operational efficiency. Flexibility will be expected. On‑call availability as needed Weekend and Holiday rotation as needed Eligible for shift differentials where applicable Position is classified as non‑exempt and eligible for overtime in accordance with labor lawsPHYSICAL REQUIREMENTS & WORKING CONDITIONS
This position involves a combination of physical activities necessary to perform essential job functions effectively. The employee should be able to: Sit, stand, and walk for varying lengths of time throughout the workday Lift, push, pull, or carry light to moderate loads, with assistance available for heavier items Use fine motor skills , including grasping, reaching, and handling equipment or materials as needed Perform routine movements , such as typing, writing, or handling supplies Bend, stoop, kneel, or crouch occasionally for job‑related tasks See and hear well enough to read documents, operate equipment, and communicate effectively Work in a dynamic environment , adapting to occasional changes in pace or setting Reasonable accommodations will be considered to support individuals in performing essential job functions.ESSENTIAL JOB DUTIES & RESPONSIBILITIES

As a member of our team, you will embody these values in your daily work by: Compassion : Demonstrating empathy and kindness in every interaction, recognizing the unique needs of our patients and colleagues Advancing : Embracing innovation, continuous learning, and striving for improvement in all aspects of patient care and team collaboration Relationships : Building trust through open communication and teamwork, fostering a supportive atmosphere that prioritizes respect and understanding Exceptional : Holding yourself to the highest standards, delivering superior care, and contributing to a culture of excellence that impacts both patients and colleagues Admission and Assessment Perform admission intake, including chief complaint, complete set of vitals, present and interim history Document allergies and type of reaction Involve patient, pharmacy and other resources to obtain accurate medication reconciliation Conduct regular and accurate patient assessments effectively (age appropriate) Recognize any deviation from normal and report pertinent findings to appropriate provider in a timely manner Maintain patient flow from waiting area to exam rooms and keep patients informed of unexpected waits Assisting Provider During Clinic (LPN Scope) Collect and prepare specimens for lab tests Handle instruments Prep patients for procedures Prepare patient information for clinic chart Perform treatments, including childhood/adult immunizations Provide patient teaching as needed during clinic visit, including family or significant other as indicated (preps of diagnostic testing, etc.) Maintain high level of EKG proficiency Enter data into the computer system per provider orders Emergency Treatment (LPN Scope) Initiate or assist in patient care in an effective and responsive manner as each situation requires Triage each emergency situation appropriately and involve other resources as appropriate Follow emergency policies, procedures and protocols in an effective manner Professional Behaviors Remain calm in emergent and stressful situations Work well as a team member, build and maintain a climate of trust Willingly accept responsibilities and cooperate with implementation of change Adapt to change and workload and be flexible changing assignments as needed Enhance professional growth and development through participation in educational programs and current literature Demonstrate self‑accountability for professional behaviors Patient Documentation Document accurate current assessment of patient status that reflects interim history, chief complaint, present history and complete vitals Keep Medication List current Document communication to patient as complete Document communication from others regarding patient as complete Promoting and Ensuring Patient Safety Follow procedures and protocols to ensure safe administration of medications and monitoring for adverse effects, including following five rights of medication administration; check allergies before administration; have patient wait 20 minutes after injection Use National Patient Safety Goals (for critical access hospitals) to ensure patient safety Implement infectious disease controls as appropriate, including isolation procedures, proper use of hand washing and gloves Follow proper procedures for disposal of sharps and hazardous materials Utilize proper body mechanics, transfer/lifting techniques and appropriate equipment to minimize fall risk to patient and injury to self Inform and involve RN regarding patient care issues in a timely and appropriate manner Effective Communication Call patients with reports in a timely manner and explain results to patients as directed by provider Communicate and collaborate effectively with other health team members regarding patient situations, satisfaction needs, and recommendations for identified outcomes Identify and recognize abnormal symptoms/changes in patient condition and appropriately report condition to medical provider Triage incoming calls from patients and obtain a full report for the provider Report deviations from normal Patient Centered Medical Home Practice Guidelines Ensure multi‑disciplinary, team approach in caring for patients and families Utilize practice guidelines to improve patient outcomes Quality Program Participation Actively participate in CRHC’s Quality Program, committees, performance improvement initiatives, and operational support activities Demonstrate commitment to Quality Improvement (QI) practices Attend department huddles regularly Complete tasks as outlined on the Department Huddle Board Other Responsibilities Perform additional duties as assigned to support the strategic initiatives of CRHC #J-18808-Ljbffr DeKalb HealthVacancy posted 1 day ago
