Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through engaging interviews and thorough evaluations of medical records, identifying emotional, social, and environmental factors that contribute to each patient’s unique circumstances.

• Provide ongoing counseling services to patients and families, addressing adjustment problems and enhancing emotional well-being while guiding them through the therapeutic journey.

• Collaborate with various rehabilitation specialists, such as physical and occupational therapists, to formulate individualized care plans that align with patients' psychosocial needs and rehabilitation goals.

• Assist patients in navigating community resources, including support groups, job training programs, or financial assistance for ongoing care.

• Educate patients and their families about available post-rehabilitation services to ensure a smooth transition back to home life or alternative care settings.

• Participate actively in discharge planning meetings, ensuring effective communication among the healthcare team and timely follow-ups for ongoing support and care.

Patient Population / Client Focus\n

Your primary focus will be on patients in a detox center, where you will address complex mental health conditions and substance use disorders. You will work collaboratively to support individuals aiming to regain control of their lives through personalized therapy and by connecting them with essential resources post-treatment. Qualifications\n

• A Master's degree in Social Work from an accredited program is mandatory.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 75,000 to 100,000 annually, commensurate with experience and qualifications.
