Registered Nurse, Home Health AccentCare seeks a Registered Nurse, Home Health to provide high-quality in‑home nursing care to patients associated with Hanford Pharmaceuticals. The RN will perform comprehensive assessments, administer medications and therapies, manage wound and IV care, and develop individualized care plans. The RN will educate patients and families on disease management and treatment adherence and document care in EHR systems. The RN will collaborate closely with physicians and pharmacists to ensure safe transitions, uphold rigorous quality and safety standards, and contribute innovation, continuous learning, and improved patient outcomes. Position Type: Registered Nurse, Home Health Coverage Area: Chester and Delaware Counties, PA Pay: $91,000 - $ / year (per‑point basis) Schedule: Full Time On Call: Yes Responsibilities Assessment of the status of nursing needs of assigned patients and overall management of the patient’s care plan Complete initial patient assessment upon admission, including appropriate patient history Select appropriate nursing diagnosis Assess medicines to identify potential ineffective drug therapy or adverse reactions, side effects, allergies, contraindicated medications Establish realistic, measurable, observable goals consistent with the patient’s diagnosis Inform physician of assessment results and recommended interventions Implement appropriate nursing interventions within scope of the Nurse Practice Act Re‑evaluate patient needs as required Document and submit assessment, progress, and discharge notes and paperwork in a timely manner Inform physician and other personnel of changes in patient condition and needs Qualifications Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation Minimum of one year of RN experience Current CPR Certification required Benefits Medical, dental, and vision coverage Paid time off and paid holidays 401(k) Flexible spending and health savings accounts Wellness offers including employee assistance program, pet insurance, and access to Calm Company store credit for first AccentCare‑branded scrubs for patient‑facing employees At AccentCare, you’re part of a community that cares — for patients and each other.
