Physical Therapist - Outpatient Pelvic Health

Job Overview We are seeking a pelvic health physical therapist to support our outpatient teams at Fairview Southdale Place Clinic in Edina and Fairview Uptown Clinic in Minneapolis! The phyiscal therapist will will provide outpatient pelvic health services at both Southdale Place in Edina and Fairview Highland Park Clinic in St Paul. Where training and experience is limited, Fairview offers mentoring and development opportunities for professional growth. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes. FTE: 1.0 (80 hours per pay period). Schedule:Monday - Friday, 8-hour days (3 days in Edina, 2 day in Minneapolis). This position will relocate to a new location August 2027. Responsibilities The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation Gathers pertinent data Performs examination Determines treatment diagnosis Develops an individualized plan of care through collaboration with the patient and family Analyzes data to identify needs and potential risks Determines prognosis for recovery Establishes individualized goals with patient and/or caregiver Individualizes plan of care considering developmental level and cultural differences Implements and modifies plan of care based on reassessment and patient response Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care Establishes appropriate discharge plan Partners with patients and families to provide skilled intervention Implements plan of care Alters treatment/plan of care to reflect change in patient status and response to treatment Assesses patient and/or caregiver learning needs and provides appropriate education Required Qualifications Physical Therapy License in the state of Minnesota or temporary license in the state of Minnesota Basic Life Support (American Heart Association or Red Cross) Preferred Qualifications Doctorate of Physical Therapy 1 year experience Pelvic Health experience Benefit Overview Fairview offers a generous benefit package including but not limited to medical, dental, vision plans, life insurance, short-term and long-term disability insurance, PTO and Sick and Safe Time, tuition reimbursement, retirement, early access to earned wages, and more! The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions.
