Overview of the Primary Care Physician Role:The Primary Care Physician is responsible for delivering comprehensive primary care services to a group of veterans, ensuring consistent care within a collaborative healthcare team. This role involves assessing and managing both new and returning patients with various medical conditions, such as hypertension, diabetes, ischemic heart disease, COPD, depression, and chronic pain. Care may be provided through in-person visits, telehealth, or video consultations, with occasional travel to VISN medical centers or Community-Based Outreach Clinics (CBOCs) as needed. The physician will also order diagnostics, consultations, medications, and other therapies as required, address preventative health needs, and complete all necessary VA documentation, including clinical reminders and screenings, promptly. Telehealth and video consultations must be conducted from approved VA sites or designated alternate locations. Additionally, the physician may assist the Call Center as required.
Key Responsibilities:Veteran -Centered Care:- Provide comprehensive primary care services focusing on health maintenance, disease prevention, patient education, and chronic disease management.
- Encourage shared decision-making with veterans and their families to achieve the best health outcomes.
- Adopt and implement the PACT model, emphasizing collaboration and coordination among multidisciplinary healthcare team members.
- Apply evidence-based practices to improve patient care and outcomes.
- Engage in ongoing quality improvement initiatives to enhance care delivery.
- Utilize various quality measurement tools to assess and enhance clinical performance and patient outcomes.
- Act as a mentor and role model for fellow healthcare providers within the primary care team.
- Participate in educational activities, offering guidance and support to residents, students, and other trainees.
- Facilitate care coordination for veterans, including specialty care, mental health services, and other ancillary services to ensure comprehensive healthcare delivery.
- Collaborate with other healthcare professionals to create personalized treatment plans for veterans.
- Leverage the VA's electronic medical record system for documentation, test ordering, and communication with patients and team members.
- Integrate telehealth as a vital component of primary care service delivery.
