Licensed Practical Nurse OCU

Location: Southern Ohio Medical Center (SOMC), 1805 27th Street, Portsmouth, OH 45662, USA General Summary Works under the supervision of the Registered Nurse. The LPN's primary job function is to assist the RN in assessing, planning, implementing, evaluating, and supervising individual patient care on a nursing unit according to unit policies and procedures. Assists the RN with implementation of plan of care. Supports each strategic value by providing excellent care to those we serve. Performs other duties as assigned. Qualifications Education: High School Diploma or successful completion of an equivalent High School Exam. Education: Graduate of an accredited school of practical nursing. Licensure: Licensed to practice as an LPN in Ohio. Licensure: Basic Life Support (BLS) within 90 days of hire. Experience: Interpersonal Skills - Ability to develop positive interactions with patients, patient families, physicians/providers, and staff; hand/eye coordination for injections, IV start, computer use; clear verbal communication. Essential Physical Requirements: Occasional lifting in excess of 100 pounds, frequently carrying 50 pounds. Stands or walks for >50% of shift; sits for 1–3 hours reviewing data and documenting. Use hands for repetitive grasping, fine manipulation, pushing/pulling equipment. Occasional bending, squatting, climbing, crawling, reaching above shoulder level. Regular contact with general public and customers. Works under close supervision or independently. Manages deadlines, irregular activity schedules, attention to detail. Essential Sensory Requirements: Good vision; clear communication ability. Exposure to Hazards: Good physical surroundings; occasional exposure to dust, odors, body fluids, toxic substances, unpleasant patient care activities, infectious diseases; frequent noise. Job Specific Duties and Performance Expectations Assists in completion of the nursing assessment of patients and documents appropriately on patient record. Assists with developing, evaluating and updating a plan of care based upon the assessment of the patient. Assists with the discharge planning process. Provides, delegates, and coordinates patient care. Facilitates patient/family teaching as appropriate. Participates in hourly rounding and bedside shift report. Answers call lights and responds to patient needs. Performs total patient care. Pre-cleans, decontaminates and transports contaminated instruments according to policy. Performs intermediate-level disinfection and/or high-level disinfection per the Instructions for Use (IFU) by the manufacturer; ensures scopes or probes are cleaned, disinfected, maintained and stored per the IFU’s.
