Harmony Care LLC is seeking compassionate and dedicated Independent Contractors to serve as Shared Living Providers (SLPs). In this role, you will open your home to an adult or child with intellectual or developmental disabilities, seamlessly integrating them into your daily family life and community. Core Responsibilities

• Assist with Activities of Daily Living (ADLs), including personal hygiene routines, nutritious meal preparation, and safe mobility support.
• Administer prescribed medications accurately and coordinate all medical, dental, and specialized clinical appointments.
• Implement Individual Support Plans (ISPs), behavioral interventions, and specific daily habilitation goals.
• Foster ongoing community integration by actively facilitating recreational activities, hobbies, and social outings.
• Maintain accurate daily electronic documentation, including progress tracking and General Event Reports (GERs).
• Ensure a safe, fully accessible, and clean home environment that strictly complies with all state oversight regulations.

Provider Qualifications

• Must be at least 19 years of age with a high school diploma or equivalent.
• Possess a valid driver’s license, reliable transportation, and active automotive insurance.
• Pass comprehensive state, federal, Adult Protective Services (APS), and Child Protective Services (CPS) background checks.
• Hold or successfully complete CPR, First Aid, and state-mandated Medication Aide certifications.
• Preferred: A professional background in education, special education, or direct clinical care to support individuals with complex behavioral and medical needs.
