RN Case Manager – Acute Care Hospital (Contract)

Start Date: 6/22
Contract Duration: Through March 2027 (approx. 40-week contract)

Requirements

• Must have inpatient acute care hospital case management experience
• 5+ years RN experience in acute care setting
• 3+ years case management experience (preferred and prioritized)
• EPIC experience required
• BSN preferred
• Case Management certification preferred
• Must be able to independently manage caseload within 1–2 weeks of training

Position Summary

RN Case Manager with inpatient acute care hospital case management experience required. Must be able to independently manage a caseload within 1–2 weeks of training.

This role focuses on discharge coordination, patient throughput, and continuity of care in the inpatient setting. The RN Case Manager collaborates with patients, families, physicians, nurses, and the broader care team to develop effective discharge plans that align with clinical needs, patient goals, treatment preferences, and available resources, while supporting patient autonomy and continuity across the care continuum.

Schedule

• Monday–Friday, 08:00–16:30
• 40 hours/week guaranteed
• No call
• Weekends/holidays not required (optional OT may be available)
• Holiday coverage appreciated but not required
