Lead Clinical Excellence, Quality & Resident-Centered Care at Hale Makua Health Services in Maui.

Hale Makua Health Services is seeking an experienced and mission-driven Director of Clinical Services to provide strategic clinical leadership across our clinical services, including Admissions, Quality Assurance, and Employee Health.

Hale Makua Health Services is a Maui based non-profit that improves the well-being of those in its care through compassionate personalized health services. We are devoted to helping Maui's Elders live their best lives. Hale Makua's Kahului location is a 254-bed nursing facility.

 

The Position:

This is a key leadership opportunity for a seasoned nursing professional who is passionate about clinical excellence, quality improvement, regulatory compliance, and exceptional care for Maui's kūpuna . The Director will partner with nursing and executive leadership to strengthen clinical standards, support survey readiness, improve outcomes, and foster a culture of safety, accountability, compassion, and continuous improvement.

Clinical Leadership & Quality

• Provide clinical leadership and guidance to nursing teams and Directors of Nursing.
• Maintain consistent clinical standards, policies, and best practices across facilities.
• Monitor resident care, clinical documentation, assessments, and care plans.
• Oversee key clinical programs such as wound care, fall prevention, infection control, and medication management.
• Guide complex or high-risk patient care situations.

Quality Assurance & Performance Improvement

• Lead quality improvement and QAPI programs.
• Review clinical data, incidents, and trends to identify areas for improvement.
• Develop and monitor strategies to improve patient safety and clinical outcomes.
• Track quality goals and performance using KPIs and other data .
• Work with clinical leaders to strengthen safety and quality programs.

Regulatory Compliance & Survey Readines s

• Lead survey preparation, mock surveys, and corrective action plans.
• Ensure clinical practices and documentation meet CMS, state, and other regulatory requirements .
• Represent nursing during regulatory surveys and support staff throughout the process.
• Keep policies and clinical practices up to date with regulatory standards.

Leadership & Organizational Excellence

• Maintain nursing policies, training, onboarding, and competency programs.
• Coach, mentor, and develop clinical leaders and staff.
• Support recruitment, retention, workforce planning, and leadership development.
• Promote teamwork, innovation, and continuous improvement across the organization.
• Build a culture of compassion, accountability, safety, and excellence .

 

Requirements:

• Bachelor's degree in Nursing or equivalent combination of nursing education and progressively responsible nursing experience.
• Valid Hawaii RN License
• Valid driver's license and reliable personal transportation for regular inter-facility travel.
• Minimum 5 years of progressive nursing leadership experience in a long-term care, skilled nursing, or post-acute care setting.
• Strong knowledge of CMS regulations and regulatory requirements .
• Working knowledge of the CMS Five-Star Quality Rating System and SNF Quality Reporting Program (SNF QRP) .
• Demonstrated experience leading CMS certification and complaint surveys with positive outcomes.
• Strong strategic leadership, communication, analytical, problem-solving, and team-building skills.
• Proficiency with Electronic Health Records (EHR) and clinical data management systems
