Our community-based model gives you the opportunity to develop lasting relationships with the individuals and families you support.\n\nYou will also be part of a collaborative team that values your clinical expertise and supports high-quality, person-centered physical therapy care.\n\nAs a Physical Therapist, You’ll…\n\nEvaluate mobility, strength, balance, gait, posture, and safety needs.\nDevelop and update individualized treatment plans.\nProvide evidence-based physical therapy in home and community-based settings.\nHelp individuals improve mobility, coordination, physical function, and independence.\nRecommend mobility aids, adaptive equipment, therapeutic exercises, and fall-prevention strategies.\nEducate individuals, families, and caregivers on safe mobility techniques.\nCollaborate with caregivers and interdisciplinary team members.\nComplete accurate and timely clinical documentation.\nParticipate in required meetings and trainings.\nWhat We're Looking For:\n\nDoctorate or Master's degree in Physical Therapy from an accredited program.\nCurrent and valid Physical Therapist license in the state of practice.\nCurrent CPR certification (preferred or required in accordance with organizational policy).\nStrong clinical, documentation, organizational, and communication skills .\nProficiency in functional mobility and gait assessments, balance and postural control interventions, adaptive and assistive equipment recommendations, evidence-based physical therapy techniques, and electronic health record (EHR) systems.\nValid driver's license, a clean driving record, reliable transportation, and willingness to travel to home and community-based service locations. Flexibility to support varying schedules and assigned service areas is required.\nExperience supporting individuals with Intellectual and Developmental Disabilities (IDD) and providing physical therapy services in home or community-based settings is preferred.\nEnglish proficiency is required.
