Qualifications Board-certified or board-eligible in Colorectal Surgery Eligible for medical licensure in Oregon and Washington Commitment to a collaborative, patient-centered approach Interest in clinical excellence, innovation, and quality improvement Licensure Medical License, DEA, BLS for the applicable state Membership on LPH Medical Staff BLS for Provider certification from the American Heart Association.
