Family Medicine Physician With Obstetrics
The Family Medicine Physician with Obstetrics (OB) will provide comprehensive medical care to patients of all ages in a rural health clinic and critical access hospital setting. This includes full-spectrum family medicine with surgical obstetrical care, encompassing prenatal, labor and delivery, postpartum care, and gynecologic services within scope of training and privileges. Responsibilities include direct patient care in the outpatient rural health clinic, inpatient care, and on-call obligations for the critical access hospital, including surgical obstetrical and pediatric call coverage. The physician will participate in quality assurance programs, peer review processes, and maintain compliance with regulatory requirements. The position also entails involvement in administrative tasks, utilization review activities, and attendance at clinic, department, and hospital meetings as required by Livingston HealthCare Medical Staff Bylaws.

- 32 patient facing hours/week
- 4 days/week clinic (scheduled between M-F, 8-5)
- 24hr OB Call (8 Shifts per Month)
Essential Functions, Duties, and Responsibilities:
1. Provide comprehensive primary care services to patients of all ages, including infants, children, adolescents, adults, and seniors, including preventive care, routine check-ups, and management of acute and chronic medical conditions.
2. On-call Critical Access Hospital Surgical OB coverage typically includes eight 24-hour shifts per month, providing obstetric call coverage, supporting labor and delivery needs, newborn care as appropriate, coordinating stabilization and transfer when a higher level of care is needed. See below regarding pediatric call responsibilities.
3. Provide full-spectrum obstetrical care, including:
- Prenatal, labor, delivery, and postpartum care
- Management of pregnancies and deliveries
- Stabilization and management of obstetrical complications as necessary
- Participation in shared call coverage for labor and delivery services
4. Perform obstetrical procedures consistent with granted privileges, including but not limited to:
- Vaginal delivery (term, preterm, post-term)
- Elective and emergent cesarean deliveries
- Labor management including induction and augmentation
- Fetal monitoring (external/internal)
- Repair of lacerations and episiotomies
- Postpartum care and complication management
- Emergency Management of Obstetrical hemorrhages
- Tubal ligations at the time of cesarean delivery
5. Provide gynecologic care including:
- Preventive women's health services
- Evaluation and treatment of common gynecologic conditions
- Office-based procedures such as IUD insertion, biopsies, and minor procedures
6. Provide inpatient care including:
- Admission, evaluation, diagnosis, and treatment of hospitalized patients
- Management of acute and chronic conditions
- Coordination of care and appropriate transfers when needed
7. On-call Critical Access Hospital pediatric coverage may include 13, 24-hour pediatric call shifts per month, depending on staffing and overall call coverage needs. Pediatric call responsibilities include newborn resuscitation, stabilization and transfer of critically ill pediatric patients, pediatric admissions with comprehensive inpatient care, and coverage of the "Kids Care Line," which supports family phone calls from the clinic. Total expected call coverage for a full-time provider is approximately eight shifts per month. In some months, OB providers may take one to three pediatric call shifts in place of obstetric call shifts to help balance departmental coverage needs.
8. Participate in on-call coverage for the Critical Access Hospital, including:
- Obstetrical call coverage
- Pediatric and newborn care, including resuscitation and stabilization
- Emergency response and inpatient management
9. Monitor patients' health status, adjust treatment plans as necessary, and provide ongoing support to promote disease control and quality of life.
10. Manage chronic conditions across the lifespan, including care of aging populations and medically complex patients.
11. Support care transitions, ensuring continuity between inpatient, outpatient, and specialty services.
12. Provide patient-centered education and counseling, empowering patients to make informed healthcare decisions.
13. Participate actively in Livingston HealthCare's quality assurance and performance improvement programs.
14. Engage in peer review and ongoing professional development activities.
15. Maintain compliance with regulatory standards, credentialing requirements, and continuing education.
16. Complete all required administrative tasks, including accurate and timely documentation for clinical and billing purposes.

Qualifications (Required):
- Medical degree (MD or DO) from an accredited institution
- Successful completion of an ACGME or AOA accredited Family Medicine residency program
- Board Certified or Board Eligible of ABFM
- Completion of required obstetrical training (including documented obstetrical experience during residency)
- Demonstrated competence in obstetrical care, including vaginal deliveries (minimum training consistent with privileging requirements)
- Valid Montana medical license or ability to obtain
- DEA registration and required certifications (e.g., BLS, ACLS, NRP)
- Ability to obtain and maintain hospital privileges in Family Medicine with OB
- Strong commitment to rural and community-based healthcare
Additional Desirable Qualifications:
- Experience in rural healthcare settings
- Experience in full-spectrum Family Medicine including OB
- Experience with procedural skills such as ultrasound, women's health procedures, and inpatient care
