We are committed to strong clinical standards, consistent operations, and long-term partnerships that support meaningful patient outcomes.Physical Therapist (PT) or Physical Therapist Assistant (PTA) – Kenesaw, NEBlue Stone Therapy is seeking a Physical Therapist or Physical Therapist Assistant to provide rehabilitation services to adult and geriatric patients at Accura HealthCare of Kenesaw in Kenesaw, NE.This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post-acute rehabilitation setting. The PT or PTA will work as part of an interdisciplinary rehabilitation team to improve mobility, strength, balance, endurance, pain management, and overall functional independence.This position will serve multiple partner communities, including Accura Healthcare of Franklin - Franklin, NE with travel between locations as needed.What You'll Do – These will be different based on the positionProvide skilled physical therapy services to adult and geriatric patients in a skilled nursing and post-acute rehabilitation settingPerform patient evaluations and develop plans of care (PT) or implement treatment programs under the direction of a supervising Physical Therapist (PTA)Deliver therapy focused on gait training, balance, mobility, strength, range of motion, endurance, and pain managementMonitor patient response to treatment and adjust interventions in collaboration with the therapy teamMaintain appropriate patient schedules per established guidelinesComplete timely and accurate documentation to support quality care and reimbursement requirementsCollaborate with interdisciplinary team members and facility staff to support positive patient outcomesWhat You Bring – These will be different based on the positionGraduate of an accredited Physical Therapy or Physical Therapy Assistant programCurrent PT or PTA license (or eligibility for licensure) in the applicable stateExperience in skilled nursing, long-term care, or post-acute rehabilitation preferredStrong clinical reasoning, communication, and organizational skillsCommitment to patient-centered care and professional standardsWhy Blue Stone Therapy?We believe exceptional care begins with exceptional clinicians.
