TravelNurseSource is working with Cynet Health to find a qualified Home Health RN in Lexington, Kentucky, 40509!

Job Title: Home Health Registered Nurse Profession: Registered Nurse Specialty: Home Health Duration: 13 weeks Shift: 5-8 hour shifts, M-F + weekend and holiday rotation Hours per Shift: 8 hours Experience: Minimum 1 year in a Medsurg floor in a hospital setting License: Must have state license in hand Certifications: BLS Certification required Must-Have: - Home Care Home Base (HCHB) experience - Home health experience Description: The Registered Nurse will be responsible for managing home health patients. The RN will case manage roughly 30 patients per week. An average of 6.5 points is expected per day. RNs are required to perform start of care, recertifications, and daily visits. Clinical support will be available 24/7 for emergencies, and from 8 AM to 5 PM for other queries. There will be no on-call duties associated with this position. The RN will be required to work weekends and participate in holiday rotations as needed. Scrubs are the expected attire, with no color requirement. The orientation period includes two weeks of visits with a work coach.
