American Traveler is seeking a licensed Physical Therapist with home health experience for a 13-week travel contract serving San Mateo County, CA.

Details

• Home health setting serving patients across Brisbane, Daly City, Colma, South San Francisco, Pacifica, and neighboring cities
• Day shift hours: 8:00 AM – 5:00 PM, with schedule flexibility based on patient availability
• 13-week contract

Requirements

• Minimum 1 year of home health PT experience required
• Current CA PT license required (license required if applicable)
• Active BLS/BCLS certification required

Additional Information

• Responsibilities include evaluating and treating patients using medically prescribed physical therapy programs to restore function and maximize patient performance
• May supervise Physical Therapy Assistants as part of caseload responsibilities
• Responsible for completing initial admission/evaluation visits and reporting OASIS scores to the Clinical Team Manager within established timelines
• Documentation responsibilities include clinical findings, skilled interventions, goals, and discharge planning
