About the Role
Washington State’s Wraparound with Intensive Services (WISe) program provides comprehensive, family-driven and youth-guided behavioral health services to youth with complex mental health needs and their families.
As a Care Coordinator, you’ll help youth and families navigate the WISe process by coordinating care, facilitating collaborative team meetings, and ensuring individualized care plans are developed and implemented. You’ll work closely with youth, families, and care teams to support meaningful outcomes and help youth thrive in their homes and communities. This role requires maintaining strict confidentiality when handling protected health information (PHI).
What You'll Do
- Prepares for Child and Family Team (CFT) meetings by collaborating with team members to develop the agenda, schedule meetings, invite all participants, and create visual aids or tools to support the meeting process.
- Facilitates CFT meetings, by guiding team discussions, fostering collaborative decision making, and serving as the central communication point for the team and families.
- Proactively contact absent team members before meetings together their input and ensure their perspectives are included.
- Cultivate an effective, inclusive team culture, engaging all members and identifying their needs to meet agency and program mandates.
- Strengthen “natural supports” by understanding family history, culture, and community resources, helping you and families connect with supportive individuals and networks.
- Distribute updated copies of the Child and Family Service Care Plan (CSCP), meeting summaries, progress documentation, and task assignments to all CFT members in compliance with WISe requirements.
- Collaborate with Youth and Family Support Specialists to identify and mobilize family support, peer support, and community resources that amplify the youth’s voice during CFT processes.
- Complete timely WISe-required documentation, including assessments, crisis planning, and care plans, in partnership with team members.
- Inform youth and families about their rights, including due process, and obtain necessary consents and releases of information.
- Advocate for diversity, equity, and inclusion within 3RT and the wider community.
- Performs other duties as assigned by the supervisor

Work Environment
This position works across a variety of settings, including clients’ homes, the office, and while traveling between client locations. The office environment includes moderate noise, standard lighting, and occasional interruptions. Home visits may involve exposure to pets, residual smoke, or other household conditions, and smoking is prohibited during client visits. The role requires frequent travel for client visits and may also include travel for annual professional development.
Physical Demands
This position requires regular standing, sitting, speaking, hearing, and use of hands for computer and phone operations. The role requires sufficient visual acuity for computer and screen-based work. During home visits, the employee may need to sit on the floor, crouch, or kneel and may occasionally need to lift or assist children when necessary. Frequent sitting may also be required while traveling between client locations.

Requirements:What You'll Bring
To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.
Level I
- Bachelor’s degree in psychology, social services, or behavioral health field
- Experience working with children and families
- Ability to work flexible hours (evenings, nights, weekends)
- Possess good written and verbal communication skills and excellent documentation skills.
- Ability to work independently and as a team member.
- Working knowledge of community resources
- Ability to represent the agency in a professional manner within the community and develop collaborative community relationships.
- Proficiency in MS Office and basic office technologies.
Level II
Level II candidates must meet all Level I qualifications , in addition to the following:
- At least 2 years of experience working with children and families
- Demonstrated experience working with culturally diverse populations and/or demonstrated life experience which qualitatively improves service delivery to Rising Strong families (i.e., diverse health perspectives, cultural and family style approaches).
- Experience working with issues of homelessness, substance abuse/addiction, mental health and/or poverty.
Certificates/Licenses:?
- Possess valid WA State driver’s license and clean driving abstract.
- Successfully pass background check applicable to position.
- Eligible for Agency-Affiliated Counselor registration required.
Level I starting at $27.30 per hour.
Level II starting at $33.60 per hour.
