Sr. Clinical Social Worker

The Division of MaternalFetal Medicine is seeking a Sr. Clinical Social Worker to serve as the core clinical coordinator within a grant-funded Collaborative Care program focused on perinatal mental health. The Sr. Clinical Social Worker functions as a licensed behavioral health clinician embedded within an interdisciplinary obstetric care team and provides population-based mental health services using evidence-based Collaborative Care principles. This position follows the AIMS Center Collaborative Care Model and emphasizes registry-based population management, measurement-based care, brief evidence-based interventions, and longitudinal care coordination. This role is not a traditional outpatient psychotherapy position and does not maintain an independent therapy panel or visit-based productivity expectations.

Specific Duties & Responsibilities

• Collaborative Care clinical services.
• Independently manage a population-based behavioral health registry.
• Provide measurement-based care using validated symptom rating scales.
• Deliver brief evidence-based behavioral health interventions (e.g., behavioral activation, problem-solving therapy).
• Conduct patient assessments and longitudinal follow-up contacts (telephone, telehealth, in-person as appropriate).
• Identify patients demonstrating treatment non-response and support stepped-care adjustments.
• Coordinate care across obstetric, psychiatric, and primary care teams.
• Communicate treatment recommendations to obstetric clinicians.
• Coordinate psychiatric case consultation and assist with implementation of treatment recommendations.

Provision of Team-Based Care

• Serve as an embedded member of OB/MFM interdisciplinary care teams.
• Participate in weekly psychiatric case review meetings.
• Maintain regular communication with obstetric clinicians, nurses, and social work staff.
• Support interdisciplinary treatment planning and care coordination.

Population Health & Registry Management

• Maintain and update longitudinal patient registry.
• Track clinical outcomes and symptom progression.
• Ensure measurement-based care documentation integrity.
• Monitor caseload trends and identify patients requiring escalation of care.

Program Implementation & Quality Improvement

• Participate in program implementation and workflow optimization.
• Contribute to continuous quality improvement initiatives.
• Assist with program evaluation and fidelity monitoring.
• Support preparation of grant reports and program documentation.
• Participate in onboarding and mentoring of future staff as program scales.

Documentation

• Document all patient encounters and care coordination activities in the electronic health record.
• Maintain accurate registry documentation.
• Complete program-related reporting requirements.

Work Structure

• Population-based caseload (typically 60100+ patients longitudinally).
• Combination of patient contacts, registry management, documentation, and team coordination.
• No standing weekly psychotherapy sessions.
• No independent billing panel.
• No visit-based productivity quotas.

Minimum Qualifications

• Master's Degree or higher in Counseling, Psychology, Social Work, Nursing, or related mental health field.
• Professional licensure in Maryland required.
• MSW from an accredited university required. LCSW-C License required.
• Two or more years of case management experience in a healthcare or public welfare setting with knowledge of HIV, substance abuse, and psychiatric illness required.
• Additional education may substitute for required experience, and additional related experience may substitute for required education beyond a high school diploma/graduation equivalent, to the extent permitted by the JHU equivalency formula.

Preferred Qualifications

• Independent clinical licensure.
• Experience in Collaborative Care or integrated behavioral health settings.
• Experience in perinatal mental health.
• Experience with population-based or measurement-based care models.
• Experience working within interdisciplinary medical teams.
• Certification in Perinatal Mental Health (PMH-C).
• Bilingual in Spanish language.
