Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on the patients’ needs, goals, and physician directives. This role includes the delegation and direction of all discipline-specific therapy services for the assigned patients. The therapist is responsible for designing functional treatment programs that facilitate an effective return to community activities. Additionally, this position serves as an educator for patients, families, students, coworkers, and self.

Responsibilities

• Completes a comprehensive evaluation of assigned patients.
• Utilizes clinical reasoning to develop appropriate treatment plans and goals focused on progressing patients towards discharge.
• Documents medical records clearly, concisely, accurately, and timely, in compliance with relevant guidelines.
• Ensures care is delivered under signed, valid physician orders.
• Participates in student program development and evaluation.
• Develops and pursues an individualized professional development plan.
• Adheres to discipline-specific State Practice Act and maintains professional licensure.
• Demonstrates awareness of and participates in performance improvement initiatives.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 2 years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license to practice as a physical therapist.
• Previous experience with the agency or Select Medical is highly desired.
