Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas.PACE ProgramThe PACE (Program of All-Inclusive Care for the Elderly) program is designed to help older adults remain safely in their homes rather than entering nursing facilities. It provides complete medical care, rehabilitation, medications, transportation, social services, and supportive therapies—all coordinated by an interdisciplinary care team. Participants receive personalized care plans tailored to their unique needs, along with access to a PACE center for social activities, meals, and clinical services. By integrating medical and supportive care under one program, PACE helps seniors maintain independence, improve quality of life, and continue living in their communities with dignity.Transportation Specialist | Team LeadEnsures that PACE participants get where they need to go safely, comfortably, and on time while providing leadership and daily support to the transportation team. This role involves driving buses, vans, or cars to transport participants between their homes, medical appointments, and the PACE Day Center while observing safety regulations and using proper transfer techniques. You’ll assist participants in and out of vehicles and buildings, help with wheelchairs and ambulatory transfers when needed, and maintain a supportive, respectful environment during trips.As a Team Lead, you’ll help coordinate daily transportation operations by assigning routes, monitoring schedules, supporting staff with questions or challenges, and serving as the primary point of contact for transportation-related issues. You’ll provide on-the-job coaching, reinforce safety and customer service standards, assist with onboarding and training new team members, and help ensure compliance with organizational policies and regulatory requirements. You’ll also collaborate closely with the interdisciplinary care team to address participant needs, communicate schedule changes, and resolve service concerns.In addition to leadership responsibilities, Transportation Specialists help keep vehicles clean and in good working order, perform basic safety inspections, and communicate any participant concerns, schedule changes, or vehicle issues to the care team. In some positions, you may also observe and report changes in participants’ health or behavior and support light duties at the Day Center as needed.
