Care Coordinator

Salary Range: $62,360-$72,000 (Pay information will vary depending upon relevant experience for the position)
Full time Exempt Monday -Friday 8:30am-5pm with flexibility when needed
EDUCATION / QUALIFICATION: Bachelor’s degree required. Master’s Degree preferred. (preferably in the area of Social Work, Psychology, Gerontology, or Healthcare Administration) RESPONSIBILITIES: You will carry a caseload of members who live independently in the community and serve as their primary point of contact, advocate, and care navigator. The work will focus on proactive holistic measures to keep members independent for as long as possible. You will be responsible for:
- Member relations: Regular home visits, check-ins, advocacy to help members achieve their health and wellness quality of life goals.
- Assessment & care plans: Conduct holistic assessments to develop and maintain personalized care plans.
- Interdisciplinary collaboration: Partner with physicians, families, and care teams on care planning, care decisions, and transitions.
- Community Outreach: Represent the program at health fairs and community events, building referral relationships and presenting the concept directly to consumers.
- Excellent customer service orientation and exemplary communication skills – both verbally and written; ability to present information in a clear concise manner.
- High degree of professionalism and a thorough understanding of confidentiality.
- Ability to prioritize and multi-task in order to meet deadlines in a fast-paced environment.
- Excellent organizational, problem-solving and follow-through skills are essential
- Agility in problem solving and an ability to anticipate and address big-picture issues.
