*DUTIES & RESPONSIBILITIES*

* *Transformative Member Engagement:* Ignite change through dynamic 1:1 online consultation and coaching sessions, employing motivational interviewing skills to build powerful, therapeutic connections.
* *Empowering Health Assessment:* Elevate members' wellness journeys with vibrant, advanced health assessments, offering guidance and education that sparks lasting transformations.
* *Holistic Care and Impactful Management:* Deliver standout routine care, blending standardized procedures with personalized touches, including physical examinations and screenings.
* *Follow-up Excellence and Precision Documentation:* Bring excitement to every member follow-up, ensuring meticulous, focused documentation that reflects their unique progress and needs.

*QUALIFICATIONS AND REQUIREMENTS*

* California licensed Nurse Practitioner or Physician Assistant.
* Masters degree from an accredited NP or PA program.
* DEA Registration Certificate.
* OIG Clearance and NPI number
* Must be able to meet and receive a background check to include criminal records clearance.
