Responsibilities Make visits to the homes of patients requiring home health physical therapy services and assume responsibility for an ongoing interdisciplinary assessment of the patient. Perform initial and comprehensive assessments on all patients in accordance with agency policies and procedures. Evaluate and assess the patient’s level of functioning by applying appropriate tests. Treat clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care. Initiate diagnostic, preventative and rehabilitative procedures as appropriate to the patient’s care delivery. Qualifications Education: Graduate of a Physical Therapist education program approved by The Commission on Accreditation in Physical Therapy Education (CAPTE), a successor organization of CAPTE, or an education program outside the United States determined to be substantially equivalent to physical therapist entry‑level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association. Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided. Licensure/Certifications: Licensed as a PT by the state in which practicing, unless licensure does not apply within state regulation. Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements. Mandatory Reporter: Child & Dependent Adult Abuse. CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification. Minimum Experience: One year experience caring for similar patient population as to be assigned.
