Remote Targeted Case Manager - Mental Health & Community Support
Serving Children and Reaching Families, LLC.
Serving Children and Reaching Families, LLC is seeking a Targeted Case Manager to deliver outpatient mental health services to clients in need. The position requires strong knowledge of Medicaid rules, the ability to work with diverse populations, and a bachelor’s or master’s degree in a related field.
