Summary Statement
The Occupational Therapist evaluates and treats residents in Independent Living and Assisted Living communities in Southwest Florida in accordance with physician orders. Interventions focus on maximizing independence, safety, and quality of life through therapeutic techniques that support activities of daily living (ADLs), functional mobility, and cognitive engagement.

The Occupational Therapist ensures high-quality, efficient therapy services by managing care throughout the treatment process and delegating appropriate components to Certified Occupational Therapy Assistants (COTAs) under supervision.

The Occupational Therapist is an integral member of the interdisciplinary team, collaborating with nursing, wellness staff, and community leadership to support residents aging in place.

Key Responsibilities:

• Respond to and initiate therapy referrals within Independent Living and Assisted Living settings
• Evaluate residents’ physical, cognitive, and psychosocial abilities, with a focus on aging-related conditions
• Develop and implement individualized treatment plans that promote independence and participation in daily routines
• Address activities of daily living (ADLs) and instrumental ADLs (IADLs) such as dressing, bathing, medication management, meal preparation, and community mobility
• Recommend and train residents in adaptive equipment, fall prevention strategies, and environmental modifications within apartments or community spaces
• Support residents’ ability to safely age in place within the community
• Provide wellness-based and restorative programming to enhance strength, balance, and functional independence
• Document evaluations, progress notes, and discharge summaries in a clear, compliant manner
• Supervise and delegate appropriate treatment interventions to COTAs
• Educate residents, families, and caregivers on home programs, safety awareness, and functional strategies
• Collaborate with interdisciplinary teams including nursing, activities staff, and case management
• Participate in care plan meetings and contribute to resident-centered outcomes
• Maintain clinical competency through continuing education, especially in geriatrics and community-based care
• Participate in quality improvement initiatives, audits, and satisfaction programs
• Assess COTA competency annually
• Perform other duties as assigned

ESSENTIAL PHYSICAL TASKS:

• Ability to move throughout community environments, including resident apartments and shared spaces
• Ability to assist residents with mobility and transfers (lifting up to 50 lbs with assistance)
• Ability to sit, stand, and walk for extended periods
• Ability to travel locally between communities as needed
• Ability to bend, squat, push, pull, and perform hands-on patient care
