This role serves as a key liaison between referral sources, physicians, payors, marketers, and internal clinical teams to support timely and compliant patient admissions.Key ResponsibilitiesReview and process home health referrals for completeness, accuracy, and appropriatenessVerify insurance eligibility, benefits, authorizations, and coverage requirementsEnsure compliance with state, federal, and payer intake requirementsObtain and review physician orders, face-to-face documentation, and supporting clinical recordsCoordinate referrals with hospitals, physician offices, community partners, and marketing teamsCommunicate effectively with payors, physicians, and internal staff to resolve missing or incomplete documentationEnter and manage referrals within Home Care Home Base (HCHB) or applicable intake systemsSupport efficient daily intake operations to meet census, quality, and service goalsMaintain positive relationships with referral sources and community partnersIdentify opportunities to improve intake workflows, service access, and patient experienceMust be available to cover some weekend hours on a rotation basisRequired Qualifications2+ years of experience in healthcare intake, referral coordination, or patient accessHome Health and/or Hospice intake experience strongly preferredExperience with Home Care Home Base (HCHB) strongly preferredSolid understanding of insurance verification, authorization, and payer guidelinesAbility to communicate professionally with physicians, referral sources, marketers, and payorsStrong organizational skills and ability to manage multiple referrals simultaneouslyDetail-oriented with a focus on compliance, accuracy, and timely processingCustomer-focused with the ability to market services assertively and tactfully
