In this role, you will provide skilled occupational therapy services to patients in their homes, helping them improve independence, safety, and quality of life. Working collaboratively with physicians, nurses, rehabilitation professionals, and caregivers, you will evaluate patient needs, develop individualized treatment plans, and deliver evidence-based interventions that support successful outcomes.This position requires strong clinical judgment, excellent communication skills, and a commitment to providing high-quality, patient-centered care in a home health setting.$75 per visitKey ResponsibilitiesConduct home-based occupational therapy evaluations, follow-up visits, 30-day reassessments, recertifications, and discharge visits.Assess patients' functional abilities, rehabilitation potential, and occupational therapy needs.Develop, implement, and modify individualized treatment plans in collaboration with the interdisciplinary care team.Establish therapy goals and communicate treatment recommendations, visit frequency, and progress updates to physicians and other healthcare providers.Provide therapeutic interventions, including:Activities of daily living (ADL) trainingTransfer and mobility trainingNeuromuscular re-educationTherapeutic exerciseCoordination and balance trainingHome safety and fall prevention educationEducate patients, families, and caregivers on treatment plans, home exercise programs, adaptive techniques, and equipment use.Assess and monitor patient vital signs as appropriate and report significant findings to the Rehabilitation Manager and primary care provider.Recommend, educate, and coordinate durable medical equipment (DME) needs, including walkers, wheelchairs, grab bars, shower chairs, canes, crutches, and prosthetic devices.Maintain timely, accurate, and compliant clinical documentation within the electronic medical record (EMR) system.Collaborate with physicians, nurses, social workers, PTs, PTAs, SLPs, COTAs, and other healthcare professionals to support patient care and discharge planning.Promote continuity of care through patient and caregiver education, discharge planning, and referral recommendations.Adhere to all agency policies, infection control procedures, regulatory requirements, and professional standards.Participate in ongoing professional development and continuing education activities.Perform other duties as assigned.QualificationsRequiredGraduate degree in Occupational Therapy from an accredited program.Current and unrestricted Massachusetts Occupational Therapist license.Current CPR certification.Minimum of 1 year of experience in home health, skilled nursing, acute care, or inpatient rehabilitation.Valid driver's license, reliable transportation, and active automobile insurance.Ability to travel throughout assigned service area to conduct patient visits.PreferredPrevious home health experience.Experience with electronic medical records (EMR) systems.Skills & CompetenciesExcellent verbal, written, and interpersonal communication skills.Strong clinical assessment and problem-solving abilities.Effective time management and organizational skills.Ability to work independently while collaborating within an interdisciplinary team.Customer-focused approach to patient care.Professional demeanor with the ability to remain calm under pressure.Proficiency in electronic documentation and technology systems.Commitment to patient confidentiality and ethical practice.Physical RequirementsFrequent standing, walking, sitting, bending, stooping, kneeling, and reaching.Regular use of hands and fingers for documentation and patient care activities.Ability to lift, push, transfer, or assist patients and equipment as needed.Ability to lift and carry up to 25 pounds regularly, and more than 25 pounds occasionally.Frequent exposure to healthcare environments and infectious materials.Travel RequirementsDaily local travel required to patient homes, meetings, and educational activities.Occasional travel to corporate offices or locations outside the assigned service area.Why Join Us?Opportunity to make a meaningful impact in patients' lives.Collaborative and supportive interdisciplinary team.Flexibility and autonomy in a home health setting.Ongoing professional growth and continuing education opportunities.Commitment to delivering high-quality, patient-centered care.Apply today to join a team dedicated to helping patients live safely and independently in their homes.
