Key Responsibilities

• Collaborate with a multidisciplinary team to ensure that all aspects of patient care are interconnected and holistic, including ongoing treatment plan evaluations and patient strategy sessions.
• Educate patients and their families on mental health conditions, outlining treatment options and imparting self-care strategies to encourage recovery and empowerment.
• Keep meticulous documentation of patient progress, including therapeutic outcomes and symptom changes, to ensure continuity of care across practitioners.
• Conduct follow-up appointments aimed at assessing treatment effectiveness, addressing concerns, and making necessary plan adjustments in a timely manner.
