Shift & Schedule: Evening Shift | Floating across units required where experienced.

Time-Off Policy: Requested Time Off (RTO) must be submitted at initial presentation (maximum 5 days; holiday dates will NOT be approved).

Position Overview & Clinical Scope:

A rural long-term care facility in western New Hampshire is seeking an experienced LPN / LVN for evening-shift coverage. Working under the supervision of the Nursing Supervisor or Director of Nursing Services (DON), the LPN/LVN manages assigned unit operations, delivers direct clinical care, administers medications and treatments, and supervises Licensed Nursing Assistants (LNAs) in accordance with state practice acts.

Key Responsibilities

• Clinical Assessment & Monitoring: Continuously observe resident mental/physical status, conduct shift reports, receive/transcribe physician orders, and accompany physicians on resident rounds (including 60-day reviews).
• Medication & Treatment Administration: Administer orders via computerized MARs/TARs, order unit pharmaceuticals, administer enteral feedings (G-tube/J-tube), manage oxygen therapy, and perform complex wound care (including Wound VACs).
• Controlled Substances Management: Manage, store, and document controlled substances per state and federal guidelines, including mandatory shift-change counts.
• Team Supervision & Leadership: Supervise, assign, and evaluate LNA direct-care activities under the NH Board of Nursing scope; assist with onboarding, in-service training, and disciplinary recommendations.
• Unit Operations & Admissions: Complete daily 24-hour logs, oversee unit infection control and safety, coordinate resident admissions/transfers/discharges, and manage family communications and consents.

Candidate Submission & Qualification Requirements:

Experience & Competencies:

• LTC Experience: 3+ years of current clinical experience, with relevant practice within the last 6 months [REQUIRED].
• Floating: Willingness to float to other clinical units where experienced [REQUIRED].
• IV Certification: Active IV Certification, or willing to obtain during onboarding [REQUIRED].
• Preferred Experience: Previous travel nursing experience and background working in rural/small facility environments [HIGHLY PREFERRED].

Licensure & Certifications:

• State License: Active, unencumbered New Hampshire or Compact LPN License active in-hand at time of submission [REQUIRED].
• Life Support: Active BLS Certification from the American Heart Association (AHA) [REQUIRED].

Health, Travel & Geographic Requirements:

• COVID-19 Vaccination: Full COVID-19 vaccination required; medical/religious declinations or exemptions are NOT accepted [REQUIRED].
• Radius Rule (No Locals): Candidates residing within a 50-mile radius of the facility will not be accepted [REQUIRED].
• Transportation & Housing Alert: Located in a very rural area with limited housing options and no public transit; candidates MUST have reliable personal transportation.
