As a Home Health Registered Nurse and Case Manager, you will deliver individualized, goal-oriented nursing care within patients' homes. This position involves utilizing the full nursing process to manage patient care plans, working collaboratively with physicians and interdisciplinary teams to achieve optimal patient outcomes and support the overall health of the community.

Key Responsibilities

• Provide individualized, goal-oriented nursing care to patients in their homes, adhering strictly to physician's orders.
• Apply the nursing process (assessment, diagnosis, planning, implementation, and evaluation) to develop and manage comprehensive patient care plans.
• Serve as the primary Case Manager for assigned patients, coordinating all aspects of care and acting as the lead discipline.
• Educate patients and their families on health management, medication administration, and disease prevention.
• Maintain accurate and timely patient records in compliance with organizational and regulatory standards.

Required Qualifications

• Active Registered Nurse (RN) license.
• Minimum of 1 year of experience in Home Health or a related acute care setting.
• Demonstrated ability to provide individualized, goal-oriented nursing care.
• Proficiency in applying the nursing process.
