Who We’re Looking For Are you a compassionate, dedicated Registered Nurse looking for an opportunity to make a meaningful impact on patients’ lives in the comfort of their homes? Well Care Health is seeking a passionate Home Health Registered Nurse to join our growing team and provide exceptional care in a supportive and rewarding environment. Key Responsibilities Assess and monitor patients’ health conditions in their home environment. Develop and implement individualized care plans. Educate patients and families on health management and self‑care. Collaborate with a multidisciplinary team to ensure optimal patient outcomes. Visit 5–6 patients per shift, driving to patients’ homes to provide skilled nursing services. Conduct initial assessment and case development of patients in all stages of life in their homes. Assess, interpret, plan, implement, and evaluate patients according to age and diagnosis. Respond to documentation requests and check email intermittently during the week. Qualifications Registered Nurse (RN) with strong assessment, skilled nursing, and documentation skills. Must be comfortable using EMR documentation with company‑issued laptops, be willing to travel throughout the service area, and be able to check email intermittently during the week.
