Limited Part-Time Pharmacist | UPMC HamotSchedule: Variable day and evening shiftsDay Shift: 7:00 AM – 3:30 PMEvening Shift: 12:30 PM – 9:00 PMRequired to work every third weekend and 2–3 holidays per yearPreferred Qualifications:Hospital pharmacy experience is preferred; however, new graduates and newly licensed pharmacists are encouraged to apply.Join the UPMC Hamot Pharmacy team in a dynamic pharmacist role supporting patients primarily at UPMC Hamot, with additional opportunities to provide services across UPMC's Northern Region hospitals, including UPMC Northwest, UPMC Chautauqua, UPMC Horizon, and UPMC Kane.This position offers opportunities to engage in a variety of clinical pharmacy services, including:Pharmacokinetic consult managementRenal dose adjustmentsIV-to-PO therapeutic conversionsTherapeutic interchangesEmergency/code response participationUPMC Hamot features a fully USP-compliant sterile compounding program and a highly automated pharmacy department, providing pharmacists with advanced tools and resources to support safe and effective medication management.Pharmacists are responsible for the provision of safe, effective, and cost-conscious pharmacotherapy while supporting both centralized and decentralized medication-use systems. They collaborate closely with multidisciplinary healthcare teams to optimize patient outcomes through evidence-based medication management and clinical expertise.In addition to direct patient care responsibilities, pharmacists serve as valued clinical resources to providers, nurses, and other healthcare professionals. Opportunities may also exist to participate in quality improvement initiatives, regulatory compliance activities, staff education, and the precepting of pharmacy students and residents.Title and salary will be determined based upon education and experience.Responsibilities:Interpret and evaluate physician orders and prescriptions to prepare, calculate, and dispense appropriate medication dosage forms for inpatient and outpatient treatment in accordance with the Pennsylvania Pharmacy Practice Act, Federal Drug Laws, and other regulatory organizations.Identify and report suspected adverse drug reactions accurately and in a timely manner.Sustain the formulary by minimizing non-formulary procurements, utilizing therapeutic interchange protocols, and promoting rational drug therapy selection.Review, maintain, and screen patient medication records and profiles for allergies, drug interactions and therapeutic duplications while entering orders and/or verifying medications.Organize daily operational duties and assist in the supervision of pharmacy technicians, pharmacy aides, student interns and other ancillary personnel involved in providing pharmaceutical care.Adhere to all regulatory requirements, professional standards of practice, and hospital quality management policies and procedures in providing pharmaceutical care.Communicate with physicians and nurses to ensure prompt clarification of orders that are unclear, illegible, or incomplete.Provide drug information and/or educational programs to healthcare professionals and pharmacy students.Utilize all available computer systems and technologies in the provision of pharmaceutical careFill prescription orders in accordance with State and Federal regulations, utilizing technological resources and equipment.Prepare and dispense intravenous admixture solutions, as required, using aseptic technique in an accepted sterile product preparation area.Graduate of an ACPE accredited College of PharmacyB.S in Pharmacy or Doctor of Pharmacy (PharmD) degreeRequired: Current licensure as a Pharmacist in practicing stateEntry Level – no previous experience required; pharmacist experience preferred.Licensure, Certifications, and Clearances:PharmacistAct 34UPMC is an Equal Opportunity Employer/Disability/Veteran
