Discharge & Care Coordination Specialist

Wight Healthcare

NHS Trust is seeking a Patient Flow Coordinator (PFC) to drive safe, timely discharge planning and coordinate care across wards, social services and community partners. In this role, you will help connect the dots to ensure every patient’s journey out of hospital is smooth and tailored to their needs. Working with a dynamic multidisciplinary team, you will coordinate information, align services, and support patients, families and staff to reduce delays and improve patient outcomes. #J-18808-Ljbffr Wight Healthcare
