Our value-based approach to care delivery aligns incentives, offering patients better outcomes, payors guaranteed reduction in medical expense, and provider partners greater financial independence.Role DescriptionAs the Atlas Oncology Partners Advanced Practice Provider, you will provide direct clinical care to patients with cancer in close collaboration with our partner oncology practices. You will support and enhance the provision of high quality, evidence-based, whole-person care by diagnosing, evaluating and managing cancer-adjacent comorbidities as well as coordinating care and communicating with existing primary care and specialty providers. You will manage a longitudinal panel of patients who are actively engaged with our partner oncology practices to optimize the opportunity for patients to achieve maximum health outcomes throughout all phases of their cancer journey.Reporting: This role will report to the Medical DirectorResponsibilities include, but not limited to, the following:Provide direct, evidence-based primary care to patients with cancer, both the office and virtuallyMaintain close collaboration with the partner oncology providers who are managing the patient's cancerCoordinate care and optimize referrals to established primary care and specialty care providers in the communityParticipate in the delivery of specialized clinical programs, including congestive heart failure, frailty, and behavioral healthProvide transitions of care visitsEscalate clinical issues to the supervising Market Medical Director when appropriateCollaborate with VP of Clinical Operations and Practice Manager to continuously improve clinical operationsEngage in achieving high scores on quality measuresParticipate in interdisciplinary care team activities, including huddles and case reviews, with other members of the Atlas Oncology Partners care teamPerform other related responsibilities as neededRequired Qualifications· Current certification as a Nurse Practitioner or Physician Assistant· At least 4 years of direct patient care experience in primary care and/or oncology· Extensive knowledge and experience in areas relevant chronic disease management, and palliative care· Strong ability to collaborate with diverse teams, build relationships, and foster a positive work environment· Comfort with change, process improvement, and working in a start-up or growing organizationPreferred Qualifications· Specific domain knowledge and/or direct clinical experience managing oncology and/or other complex, high acuity patients· Proficiency in coordinating and collaborating with various healthcare providers and interdisciplinary teams to ensure seamless care transitions, effective communication, and integrated care for patients· Excellent verbal and written communication skills to effectively interact with patients, families, healthcare providers, and other stakeholders· Experience working in a rapidly growing, Value Based Care (VBC) environmentSome of the benefits of our comprehensive benefits package includes:Health, Dental, Vision insuranceCompetitive payPaid vacation, holidays, and sick leaveYou will collaborate with like-minded health care professionals who, like you, understand the importance and value of Atlas' high-quality, value-based care model.Atlas is an equal opportunity employer that is committed to inclusion and diversity.
