The Hamlet Rehabilitation and Healthcare Center at Nesconset Day Care is seeking a compassionate and dedicated Social Worker to join our interdisciplinary team. The ideal candidate will provide emotional support, care coordination, discharge planning, and advocacy services to residents and families while promoting overall well-being and quality of life.

Responsibilities

• Provide psychosocial assessments and ongoing support for residents and families
• Assist residents with adjustment to facility placement and care plans
• Coordinate discharge planning and post-discharge services
• Advocate for resident rights and ensure resident-centered care
• Collaborate with interdisciplinary team members to support resident needs and goals
• Maintain accurate and timely documentation in accordance with facility and regulatory requirements
• Assist with care plan meetings and family conferences
• Provide crisis intervention and emotional support as needed
• Ensure compliance with all federal, state, and facility regulations
• Participate in quality assurance and survey preparedness activities

Qualifications

• Bachelor's Degree in Social Work required; Master's Degree preferred
• Previous experience in a skilled nursing facility, rehabilitation center, healthcare, or long-term care setting preferred
• Strong communication, organizational, and interpersonal skills
• Knowledge of discharge planning, resident rights, and psychosocial services
• Ability to work effectively within a multidisciplinary team
• Compassionate and resident-focused approach to care
