Job Description
Title
Long-Term Care Nurse

Schedule
Block scheduling; Weekend coverage: Every other weekend.

Assignment Details
Avg. daily census: 42 patients; 42 beds.

Requirements
- NY license required in hand to submit.
- Minimum 1 year of clinical experience.
- EPIC charting experience required.
- Patient-ratio experience: 1:10 (days).

Roles & Responsibilities
- Assist patients with activities of daily living (ADLs) including bathing, feeding, and personal care.
- Perform safe patient transfers and assist with mobility using various equipment.
- Monitor skin integrity and implement wound prevention measures.
- Provide routine catheter care and assist with continence management.
- Assess and record vital signs, observing changes in patient conditions.
- Adhere to infection control protocols and maintain a safe environment.

Compliance / Notes
- Orientation: 16 hours, held during the day the first week.
- Candidates within 75 miles of the facility are not accepted.
- Travelers will be expected to float per facility needs.
- Pre-approved time off: one occurrence.
