Our residents share a bond of mutual respect with our staff that builds memorable and lasting relationships.OverviewUnder the direction of a licensed nurse, a certified nursing assistant performs daily personal care. Their primary responsibility is to ensure the residents' quality of life by meeting their psychosocial needs in a safe and clean environment.Essential FunctionsProvides personal hygiene (bath, shower, shampoo, shaving, dressing, elimination needs, facial hair clipping, nail clipping, and other personal care).Performs safe and proper transfers and assists with ambulation by using assistive devices such as gait belts, walkers, wheelchairs, and mechanical lifts.Assists with restorative activities as identified in individual resident care plans. These may include ambulation, range of motion exercises, and assistance with activities of daily living (ADLs) designed to maintain or improve the residents' functional abilities.Accurately weighs residents on admission or as assigned in a timely fashion. Takes vital signs and responsibly reports observations and changes in resident condition.Checks bedfast or incontinent residents at least every two hours to adjust position, toilet and provide incontinent care.Provides for physical comfort and safe environment. Follows all safety and security policies and procedures; performs all tasks to assure resident and personal safety and the protection of co-workers and facility property.Assures resident rooms are in a neat, orderly fashion while maintaining the resident's right to privacy, i.e., neat closets, labeled personal items. Assures cleanliness of utility rooms, kitchens, staff rooms, shower rooms, linen rooms and other work areas.Promptly and courteously answers residents' call bells and other requests for assistance; follows residents' plan of care regarding out-of-bed schedule and respects residents' choices for going back to bed unless otherwise ordered by Physician or Charge Nurse.Serves meal tray, assists feeding as appropriate, and provides nourishments to residents as assigned in a timely fashion.Completes documentation in EMR in an accurate and timely fashion.Communicates with nurse any changes in resident condition, particularly behaviors, bowel regime, and skin conditions.Follows all policies and procedures, annual mandatory in-services, required meetings, and all other conditions of employment are consistently met.Performs other duties as assigned.Education and ExperienceHigh school diploma or equivalent is required.Current certification as a Certified Nursing Assistant in the State of Connecticut is required.Experience with MatrixCare or other electronic medical records is preferred.Must be able to speak, read, and write the English language in an understandable manner.Environment and Working ConditionsAbility to focus on task needs and perform intellectual executive functions as related to documentation, charting, care device operations, etc.Ability to execute intellectual tasks and duties under conditions of high demand and distraction.Ability to complete the essential functions of the position with or with adaptive personal devices (e.g., hearing aid, corrective lenses).Ability to operate equipment and technology required for the position.Able to wear personal protective equipment.Must be able to communicate effectively.Able to assist with the evacuation of residents under emergency status to the best of their physical ability.Able to function independently with integrity and work effectively with residents, personnel, and support agencies.Able to constantly stand or walk to provide resident care and assistance.Able to frequently push and/or pull a force up to 30 pounds including residents weighing up to 250 pounds in wheelchairs and geri-chairs.Able to reach overhead, bend, and carry or lift 50 pounds occasionally and 35 pounds frequently.Able to frequently transfer and support residents weighing up to 250 pounds while positioning, and transferring residents to beds, toilets wheelchairs, etc. Additional employees or mechanical assistance may be used.Able to frequently reposition residents weighing up to 250 pounds in bed for assistance with treatments, hygiene, and comfort needs. Additional employees or mechanical assistance may be used.HoursFull-time, 3rd shift: 11:00pm-7:30am. 32 hours per week, every other weekendPay$20.00 - $24.00 per hour (based on years of experience) + shift differential (Weekday nights: $1.50 p/h, Weekend nights: $2.00 p/h)What are the benefits?Working in a Life Plan community will bring out the best in you.
