Implement and monitor therapeutic interventions to improve self-care, mobility, cognition, and functional independence. Administer standardized assessments, document progress, and adjust treatment plans as needed. Collaborate with an interdisciplinary team to coordinate care and support discharge planning. Educate patients and families on home exercise programs, adaptive equipment, safety strategies, and activity modification. Maintain thorough and timely documentation in the electronic medical record, ensuring compliance with regulatory standards.
