This position involves care navigation for adult and geriatric patients. Key responsibilities include admission criteria assessments, care coordination, and discharge planning. The nurse will manage appeals and denials, determine medical necessity per evidence-based guidelines, and conduct needs assessments. Experience in disease management and order durable medical equipment is required. Working with interdisciplinary teams such as pharmacy, respiratory services, and social services is essential. Knowledge of regulatory guidelines including HIPAA, OSHA, and the Joint Commission is expected. The Registered Nurse may float to other units as needed. The daily patient census averages around 100, with staffing ratios of 1:16 during the day. Weekend coverage is required as part of the contracted hours. Candidates must demonstrate proficiency in the electronic documentation system. Travel experience is preferred, but not mandatory. Returning staff must have been away for at least one year. Travel pairs may be submitted but must work in different roles. All candidates must comply with necessary health requirements, including vaccination. Familiarity with the acute hospital environment is essential for this role.
