Licensed Practical Nurse - Nurse CoordinatorAlexandria, VA Pay From: $33 per hourMUST:Experienced Licensed Practical NurseMust have an active Licensed Practical Nurse in VirginiaMust be BLS certification2+ years of experience as a Licensed Practical Nurse2+ years of experience working with specialty in Pediatric, Behavioral Health and/or Family Medicine2+ years of experience with healthcare insuranceAbility to work effectively with a diverse workforce and patient baseAbility to work in a fast pace environmentAttention to detail and excellent technical skillsDUTIES:Facilitates daily flow of medical care in clinicPerforms clinical duties, including phone or in-person triage, nurse visits for chronic care education and management, and communication of clinical informationWorks with multidisciplinary team, to include the medical provider, behavioral health provider, care coordinator, and medical assistant to provide high quality, coordinated, and patient-centered primary careManages high risk patient care, including patients with multiple co-morbidities or those at high risk for hospitalization or readmissionsResponds to patient phone calls, questions, and requests for care, in coordination with medical patient care teamInforms patients of lab and diagnostic results and need for follow up careAdministers medications and immunizationsProvides education to patientsAssists with ordering and maintenance of clinical supplies, onsite medications, and equipmentObtains prior authorization for medications and urgent patient proceduresAssists with administration of Virginia state vaccine program by performing vaccine inventory and assisting with ordersAlong with care coordinators, assists patients in efficiently navigating the health care systemParticipates in clinic quality improvement and risk management activities involving individual patient outreach, population health campaigns, and chronic care managementTracks patients who need follow up GYN procedures and diagnostic tests, coordinates receipt of results, and assist patients with scheduling procedures off siteTracks patients with abnormal cancer screening studies to ensure follow up care is providedFills in for medical assistant duties as neededPrepare and maintain special internal and external reports as requested by Supervisor and/or Department HeadMay be assigned to various project lead roles to advance the development of the department and support ad hoc needsAssists with developing, coordinating, and/or recommending improvements in program areaMay be asked to assist during weekend or eveningAble to work under minimal supervisionPerform additional duties and take on other responsibilities as dictated by events and circumstances*Quadrant, Inc. is an equal opportunity and affirmative action employer.
