Quality of Life Engagement Specialist

Role Title: Quality of Life Engagement Specialist Location: Client homes throughout the greater Sacramento region, including Davis, Woodland, Sacramento, Roseville, Rocklin, Lincoln, and surrounding communities. Candidates should ideally reside in central or north Sacramento, or within the service area along the I-80 corridor. Hours: Part‑time hours with a gradual ramp‑up from approximately 10 hours per week based on client needs and caseload growth. Variable schedule; hours cannot be guaranteed. Potential to grow into a full‑time position over the long term. This role is ideal for a creative, dynamic, and self‑driven therapeutic arts or recreation professional with wide ranging experience in delivering quality of life services within the field of aging. Passionate about working with older adults and adults with disabilities; a tenacious out‑of‑the‑box thinker and problem‑solver, bringing optimism and commitment to excellence. Responsibilities Direct Service: Planning and Delivery of Creative, High Quality Hummingbird Services Conduct initial phone calls with prospective clients and community members, demonstrating a holistic understanding of the client’s quality of life interests and needs and clearly articulating how the program can be customized. Conduct in‑home assessments of new clients to guide the formulation of creative, customized Quality of Life Plans. Develop written Quality of Life Plans focused on creativity and joy, shared with client and/or responsible party. Implement Quality of Life Plans, obtaining materials, identifying barriers, and strategizing solutions. Regularly develop new activities reflecting best practices and targeted to optimal individualized outcomes. Update and modify plans to meet changing needs and introduce new ideas as appropriate. Establish positive relationships and communicate effectively with clients, responsible parties, and family dynamics. Develop curricula for group settings, including assisted living, caregiver support groups, and care provider training. Plan and deliver Sage’s Home Care Quality of Life Programming: monthly consultations and training with Personal Care Assistants, provide written instructions, and prepare monthly reports summarizing activities and outcomes. Documentation and Communication Maintain up‑to‑date documentation, including engagement plans and summaries of activities delivered. Communicate regularly with elder care professionals—care managers, fiduciaries, family members, care providers, home care staff—to advocate for clients’ needs and outcomes. Community Relationship‑Building Collaborate with supervisors to build relationships in the community, elevating the program’s profile among professionals. Represent the program in community and professional forums and maintain an active contact list. Contribute to Team Knowledge Share knowledge, support, and guidance in monthly team meetings and through workshops. Present new knowledge and best practices to enhance service delivery. Knowledge, Skills, and Abilities Professional guidance grounded in extensive knowledge of aging, arts, and therapeutic activity best practices. Deep understanding of aging processes, conditions affecting older adults and adults with disabilities. Knowledge of clinical mental health issues in the aging population, including depression, dementia, substance abuse, sleep and eating disorders, and assessment of self‑harm risks. Judgment in mandatory reporting and legal matters. Skill in developing holistic quality of life assessments and plans, incorporating resources and creative solutions. Curriculum development and training skills. Navigate complex family and professional dynamics to achieve positive outcomes. Resourceful use of local and eldercare resources. Support clients and teams from a strengths‑based perspective, inspiring others. Integrity, honesty, and commitment to high standards of professional ethics. Collaborate with multidisciplinary teams. Excellent listening and written communication skills. Prioritize work and manage time effectively. Proficiency in Microsoft Office Suite, Google Drive, Google Chrome, Zoom. Qualifications Mental health professional or comparable training in therapeutic arts, including a credential in music, art, recreation, drama, or expressive arts therapies OR a bachelor’s degree and 10 years of experience working with older adults using art and recreation. Minimum of 5 years of experience working with older adults. Empathetic and compassionate, with a creative approach to individualized activity plans. Proficient in holistic quality‑of‑life, life enrichment, and engagement practices. Exceptional interpersonal skills to engage a diverse range of individuals. Excellent written communication and time‑management skills. Experience with G‑Suite and video platforms such as Zoom and Google Hangouts.
