Assignment Details:

Contract Length: 8 weeks
Guaranteed Hours: 40

Requirements:

Current state license as a Registered Nurse
Homecare Homebase Software Experience
OASIS Experience
1-2 years experience as a Registered Nurse in a clinical care setting
2+ years Home health experience
Proof of current CPR certification
Current driver's license
Must have reliable transportation and insurance

Roles & Responsibilities:

Assess and respond to the needs of patients and families in home health and hospice settings.
Develop, document, and evaluate Plans of Care (POC) to ensure quality care and continuity.
Educate patients and families about care, medications, and home safety tailored to their specific needs.
Supervise and provide clinical direction to home health aides to ensure quality service.
Monitor compliance with third-party payer requirements for assigned cases.
Maintain effective communication with patients, families, and interdisciplinary team members.
