Home Care Coordinator

We are seeking a compassionate, organized, and dependable Home Care Coordinator to join our team. The Home Care Coordinator plays a key role in ensuring our clients receive safe, reliable, high-quality care in the comfort of their own homes.

This position coordinates services between clients, caregivers, families, service coordinators, and community providers while supporting both client satisfaction and agency compliance.

The ideal candidate is an excellent communicator who can multitask, solve problems, manage schedules, and build positive relationships with clients and caregivers.

• Conduct initial client assessments and identify individual care needs.

• Develop and maintain individualized care plans in collaboration with clients, families, and service coordinators.

• Monitor client needs and update care plans as appropriate.

• Maintain regular communication with clients and families to ensure satisfaction and address concerns promptly.

• Coordinate with service coordinators, healthcare providers, hospitals, and other community resources as needed.

• Assist with transitions of care and changes in client services.

• Schedule and assign caregivers based on client needs, staff availability, qualifications, and geographic location.

• Monitor daily service delivery and respond quickly to scheduling or staffing changes.

• Communicate schedule changes, client needs, and important updates to caregivers and clients.

• Monitor client service authorizations and ensure they remain current.

• Assist with recruiting, onboarding, and training caregivers.

• Provide ongoing guidance, coaching, and support to caregivers.

• Monitor caregiver performance and address concerns in accordance with agency policies.

• Ensure caregivers are informed of their assigned clients’ care plan needs and applicable service requirements.

• Promote a positive, respectful, and professional work environment.

• Maintain accurate and timely client and employee documentation.

• Ensure services are delivered according to individualized care plans, agency policies, and applicable Pennsylvania regulations.

• Maintain confidentiality and comply with HIPAA requirements.

• Monitor service utilization and staffing documentation.

• Participate in quality assurance and agency improvement initiatives.

• Prepare reports and documentation as needed.

• High school diploma or equivalent.

• 2+ years of experience in home care, healthcare, case management, scheduling, or a related field.

• Strong organizational and time-management skills.

• Excellent written and verbal communication skills.

• Ability to manage multiple priorities in a fast-paced environment.

• Strong problem-solving and decision-making abilities.

• Professional, compassionate, and client-focused approach.

• Basic computer skills and ability to learn electronic health record and scheduling systems.

• Experience in home care, personal care services, or healthcare coordination.

• Previous supervisory, team-lead, or staff-coordination experience.

• Familiarity with Pennsylvania home care regulations and documentation requirements.

• Experience working in a fast-paced office setting.
