Triage Rn Opportunity At Healthpartners North Suburban Family PhysiciansHealthPartners North Suburban Family Physicians has an exciting opportunity for a Triage RN to join our team! This.80 fte position will be based at our Lino Lakes clinic with potential for coverage at our Roseville or Hugo clinics.This position will work in clinic Monday, Tuesday, Wednesday, Friday days (start time between 7:45am – 8:30am and end time between 4:15pm – 5pm).This position provides triage support for our clinic at the North Suburban Family Physicians clinics by using protocols embedded in the electronic medical record and a variety of resources including standing orders and protocols. This position also includes occasionally working with patients that have questions or needs once on-site. In addition, this role provides comprehensive assessments to establish and evaluate a plan of care and address changes in a patient's condition, delegates nursing tasks when appropriate and provides supervision and monitoring of function or activity, creates and implements dynamic teaching plans based upon identified patient need and acts as a resource for other staff.Required Qualifications:Graduate of an accredited school of nursingCurrent RN licensure in MinnesotaCurrent RN licensure in Wisconsin (within two months of hire)Current BLS certification for health professionals through the American Heart Association or Red Cross required and maintained. On-line BLS certification is not acceptable.Strongly demonstrated written and oral communication skillsPreferred Qualifications:One-year full-time employment as an RNTriage or Care Management experience within the last three yearsAccountabilities include the following:Nursing Process:Nursing assessment is accomplished through interviewing, observation, and/or nursing procedures.Problem identification defines the impact of health problems affecting individuals and families. It assists patients/families to optimally manage their health care, respect their culture and values and helps to identify individual needs, health goals and treatment preferences.Performance is up to standard when:Uses health coaching techniques to elicit patient goals and priorities to develop individualized patient action plansUses population management tools such as registries to identify high risk patients and track, monitor and follow-up with their careEngages, assesses readiness to learn and educates patients and families to implement strategies to reach a desired goal or care interventionIdentifies and secures patient services needed for patient careInforms and supports patients and families to assist them in making informed decisions and navigate the healthcare system.Education:Education imparts knowledge designed to develop skills, to enhance decision-making, and to promote understanding.
