Discharge & Care Transitions Case Manager

Duke Health Lake Norman Hospital in Mooresville, NC seeks a case management专业 to coordinate discharge planning and care transitions for a designated caseload. You will ensure optimal use of resources and compliance with CMS guidelines while collaborating with the health care team. Required BSN or MSW, NC RN licensure (or valid compact) and Case Management Certification within 3 years. Three years of relevant experience is expected.
