About the Assignment:
Join a dedicated hospital-based Home Health department, where you'll deliver compassionate, patient-centered care in a supportive environment. The department provides comprehensive orientation and supplies all necessary equipment. You'll work with a caseload of 7-8 patients daily, with mileage reimbursed at $0.76 per mile. On your scheduled weekends, you will have Friday and Monday off, working only every other weekend, with a requirement to work 50% of all holidays.

- License/Certs: Valid Louisiana RN license, BLS, NIHSS
- Experience: Minimum 1 year of Home Health, Med Surg, or Hospice experience
- Hourly Pay Rate: $63.65

- Provide skilled nursing care to patients in their homes, focusing on wound management, heart failure, diabetes, IV antibiotics, Foley catheter, and PICC line maintenance
- Administer medications and treatments per physician orders
- Educate patients and families on disease management and health maintenance
- Document patient care using Meditech Home Care EMR system
- Collaborate with the healthcare team to develop and adjust care plans
- Travel within a 50-mile radius to designated patient locations
- Support and serve as an educator within the department
Required:
- Valid Louisiana Registered Nurse license
- BLS and NIHSS certifications
- Drivers license and reliable transportation
- At least 1 year of recent Home Health, Med Surg, or Hospice experience
- ER and ICU experience
- Experience with IV pumps and infusion therapy
- Strong general medical-surgical skills
