The Role \n

• Provide comprehensive primary care for adults 65+
\n
• Focus on preventive medicine and chronic disease management
\n
• Practice within a collaborative, team-based care model
\n
• Build meaningful patient relationships with smaller daily volumes
\n
• Help improve outcomes while reducing unnecessary hospitalizations
\n

Ideal Candidate \n

• Board Certified/Eligible in Family Medicine or Internal Medicine
\n
• Passion for caring for older adults
\n
• Team-oriented physician with strong communication skills
\n
• Value-Based Care experience welcomed but not required
\n
• Motivated to provide relationship-centered, quality care
