Schedule: PRN – Monday‑Friday – Flexible weekday hours. Service Area: Greater Nashville Area – northern focused. About the Role Physical Therapist – Home Health – PRN position. Serves patients across the Greater Nashville Area, delivering evidence‑based PT services in the comfort of patients' homes. Deliver care according to individualized treatment plans developed by supervising PTs. Promote patient safety, mobility, and functional independence. Educate patients and caregivers on exercises, mobility strategies, and home safety. Accurately document clinical notes and patient progress in EMR systems. Communicate effectively with interdisciplinary team members. Travel independently to patient homes within designated service areas. Analyze comprehensive assessment information to determine rehabilitation diagnosis. Collaborate with stakeholders to establish the plan of care, coordinate and implement care delivery while anticipating patient needs. Apply appropriate evidence‑based interventions, re‑evaluate performance and assess progress toward goals. Employ strategies to promote successful return to home, community, and occupational environments. Required Qualifications Active Physical Therapist (PT) license in the state of Tennessee. Minimum 1 year of PT experience. Strong communication and assessment skills. Ability to work independently with a flexible PRN caseload. A valid driver’s license, reliable transportation, and satisfactory driving record. Preferred Qualifications Home Health experience. Experience with EMR documentation systems. Strong understanding of rehabilitation principles and functional mobility. Training & Orientation Schedule New hire training: Held on Mondays. Clinical orientation: Conducted 3 days per week during the onboarding period (6–8 weeks, depending on experience).
