Responsibilities: Provide skilled nursing care under the direction and supervision of a Registered Nurse and physician. Assist in implementing individualized care plans and documenting patient progress and outcomes. Monitor patient conditions and report changes to the RN Case Manager and interdisciplinary team. Educate patients and caregivers on medications, treatments, and disease management. Perform skilled nursing procedures using standard homecare equipment and supplies. Maintain accurate and timely clinical documentation and reports. Communicate effectively with physicians, healthcare teams, patients, and families. Participate in regular case conferences and care coordination activities. Promote patient safety, comfort, and independence in the home setting. Qualifications: Passionate about providing exceptional home healthcare. 1-2 years of recent LPN experience preferred (Home Health, Med-Surg, Long-Term Care, Rehab, or Skilled Nursing). Active LPN license in Maryland. Current CPR certification.Experience with wound care, medication management, and patient education preferred. Strong organizational and communication skills. Basic computer proficiency (Homecare Homebase EMR experience a plus). Valid driver's license, reliable transportation, and an acceptable driving record.
