RN - Case Manager
Providence Holy Cross Medical Center is seeking an RN - Case Manager for a travel assignment in Mission Hills, California. The ideal candidate will have 2 years of acute care hospital case manager experience performing care coordination and discharge planning. This position requires no floating and offers a schedule of 5 shifts per week from 8:00 AM to 4:00 PM. The contract duration is 13 weeks, starting ASAP.
Key Responsibilities:
- Perform care coordination and discharge planning
- Assess patients and develop treatment plans
- Organize and participate in family meetings
- Competent with arranging DME, SNF, and ambulance transport
- Knowledge of drug/alcohol abuse treatment options, patients experiencing homelessness, patients with mental health history or in crisis, advance directive, POLST, protective services process, etc.
Requirements:
- 2 years of acute care hospital case manager experience
- CA state license or pending license accepted
- Must be able to perform tasks independently
- Proficient in Microsoft applications and computerized documentation
Travel Compliance:
- Must commit to working 3 of the 5 holidays (Thanksgiving, Xmas Eve, Xmas Day, NYE, NYD)
- References required: 2 references covering 1 year within the last 3 years
- Local radius: 50 miles
- Guaranteed hours: 40 per week
