Responsibilities
- Conduct psychosocial assessments for patients and families in the inpatient, acute-care setting.
- Develop and implement comprehensive discharge planning in collaboration with the multidisciplinary team.
- Provide counseling, crisis intervention, and support for patients and families regarding goals of care, treatment options, and available resources.
- Coordinate social work services, referrals, and community resources to support safe and timely patient transitions.
- Document assessments, care plans, progress notes, and discharge plans in the patient record with accuracy and timeliness.
- Educate patients and families on resources, benefits, and available community supports to promote informed decision-making.

Requirements
- Active Georgia Licensed Clinical Social Worker (LCSW) license
- American Heart Association BLS certification
- Minimum 2 years of experience in medical social work within the specialty
- Acute-care inpatient setting experience
- Ability to work the listed schedule and environment
